Working with Families

We greatly value input, feedback and involvement from families and strive to create open, honest, transparent, supportive, and communicative relationships. We see families as a key resource during the Support Planning process to ensure that we can access information and advice, including the finer details that only families have knowledge of. We encourage ongoing input to ensure we are all working collectively and effectively and see families as a crucial part of the team.

Community Activating

eLIVEate can also provide support for people living in their own homes within the community, who may require a lower level of assistance. This may include support with maintaining tenancies, finances and budgeting, shopping, cooking and daily living skills, employment and getting involved in the community. We support people with a proactive approach, with the aim of assisting them to become as independent as possible and reduce reliance on paid support where possible.

We aim to look at ways of breaking the mould of traditional “time and task” support and build links and initiatives within local communities that can assist to reduce reliance on paid support and help people to become more confident, active and valued within their community.

We currently provide these services in the Ivybridge and South Hams area.

Connected

eLIVEate offer a range of modernised Day Opportunities through our “Connected” projects. We opened our first Connected hub, based at The Clay Factory in Bittaford in January 2022, shortly followed by our Plymouth hub in April 2022.

Our Clay Factory hub has a creative focus, designed to link in with the artistic community based onsite, and provide opportunities for the people we support to exhibit their work at the regular art events and festivals held at the Clay Factory. We also work closely with The Clay Factory community and other local organisations to create opportunities for the people we support to develop work-based skills which may lead to voluntary or paid employment. The Clay Factory offers a calming and therapeutic environment and provides opportunities for inclusion and collaboration with other local organisations.

Our Plymouth hub, based at Faringdon Road in central Plymouth, offers a varied programme of sessions, aimed at developing skills and confidence within a vibrant social environment. We run a weekday Personal and Social Development programme which incorporates a wide range of independent living skills-based activities, cookery sessions, art & enterprise, and health and fitness opportunities. We work closely with local SEN schools and colleges to facilitate effective transitional support for those leaving education, or for those requiring support alongside college placements.

Active Support

Once support with eLIVEate has commenced, we will provide support as agreed and set out in each person’s Support Plan, working with an Active Support ethos to assist each person to become as independent as possible. We strive to ensure that we support people to develop their interests, passions, skills and independence, whilst ensuring that their safety, happiness and wellbeing remains paramount. We conduct regular reviews to ensure that we continue to meet needs as they change and are committed to maintaining effective and communicative relationships with families, circles of support and colleagues within health and social care to create an open, collaborative and positive environment.

Our staff teams receive comprehensive training, geared around individual needs, to ensure that we provide consistency, positivity and continue to strive to assist each person to live their life to the full and be the best that they can be.

Transition

We work closely with the person and their circles of support to develop a suitable transition plan to begin the process of setting up a new home, or in circumstances where we may be chosen to replace existing care. This may include working with schools and colleges for young people in / leaving education, with other care settings for people moving on from residential homes, foster care settings or hospital, with other providers for people wishing to transfer their current support, and with families for people moving on from the family home. This process may involve support to source appropriate housing, identifying future house sharers or a period of assessment / skills building before moving on. We will work with our housing partners, local landlords and housing associations to find the most effective solution for each person.

The person supported and their circles of support will have the opportunity to be involved in the recruitment of a staff team during this period, to ensure that skills and profiles are matched effectively. eLIVEate can also provide support and guidance for families with things such as Deputyship, Court of Protection, Universal Credit, and tenancies during this process.

During this process, we will continue to work with the individual and their circles of support to create their Person-Centred Support Plan and a range of other tools to ensure that needs and wishes have been accurately captured and documented.

Referral, Assessment and Person-Centred Support Planning

We complete a thorough referral and assessment process, working closely with the individual, their family, care managers and other professionals to build an accurate picture of the person’s support and housing needs. This will include creating a PATH, a tool which maps out a unique journey based upon the wishes, aspirations, and goals of the individual. This process will contribute to the creation of an in-depth Person-Centred Support Plan and identify the skills and profiles required ahead of recruiting and training a dedicated staff team.

Support Plans are designed around individual needs, preferences, goals, and dreams to ensure a fully person-centered ethos is implemented. Each plan is based around “The Keys to Citizenship”, a movement created by Doctor Simon Duffy from the Centre of Welfare Reform in 2014.

This involves a focus on how we support people across the seven key principles of freedom, purpose, money, home, help, life and love.

The creation of each plan will start with a facilitator gathering specific information from the person and the people who know and care about them, focusing on the person’s capacities and not their deficits. We will then focus on what support each individual needs, rather than assuming that people need to change. This shared understanding about the person will reflect what is important to them in their day-to-day life and in the future they desire.
